PACKAGE # 2 - $399 (All of Package # 1) and a stress echocardiogram to assess: 1. The likelihood of having coronary heart disease now 2. Cardiovascular and pulmonary fitness levels. 3. Heart function. 4. Valvular status. |

PRE-EVALUATION INSTRUCTIONS (For Package # 1 & # 2) 1. Please fast for 8 hours before your visit (water, black coffee/clear tea OK). (For Package # 2) 2. Do not take any medications which blunt the heart rate response to exercise 3. Wear comfortable walking shoes/clothing. |
